Review the complete claim story

A useful review looks beyond individual code accuracy. It considers whether diagnoses, procedures, modifiers, units, and documentation present a consistent account of the service delivered.

Separate isolated errors from patterns

Sampling across providers, locations, and service types can reveal repeat issues. Pattern-based findings are more actionable because they can inform education, templates, and pre-bill edits.

Close the feedback loop

Share findings in clear language with coding, billing, and clinical teams. Assign corrective actions and revisit the same measures later to confirm that the change was effective.

When the record does not support a code or modifier, route the issue through the practice’s approved query process. A reviewer should not infer clinical facts or replace the treating provider’s responsibility for complete documentation.
